New Patients
The best way to schedule an appointment is to call me. If we do not pick up immediately, leave a message including your phone number and the best time to reach you. We check messages often during business hours and will get back to you quickly.
Initial evaluation: $300
Follow up visits:
- 45 minutes: $175
- 30 minutes: $150
- 15 minutes: $100
If I am prescribing a medication for you, I will try to make sure you have enough of a medication supply to last until your next scheduled appointment. If you are running out of meds and do not have a scheduled appointment, please anticipate it is time for us to meet again. There is no charge for pharmacy call-ins during the appointment time itself. There is a $20 charge to have medications called in to pharmacy outside an appointment.
I am not a provider for individual insurance plans and I do not bill insurance companies myself. If you would like me to provide you information to complete a billing form there is no charge for that, nor is there a charge for me to complete a billing form during a session, or to provide you with a receipt to send to your insurance company. If there are insurance forms that I must complete myself outside of an appointment, there will be a charge at regular office rates for that service.
Regular fees are charged for appointments unless canceled with 48 hours notice. There is no charge to reschedule an appointment prior to 48 hours before the appointment itself.
If you are an established patient you should schedule a 30-minute visit and be sure to bring the paperwork that needs to be filled out with you. I will do my very best to complete the evaluation and paperwork in that period of time. If you are not already an established patient, it would be best if you contact me by phone before your initial evaluation and I could give you my best estimate as to the time and cost involved.
There is no charge for paperwork to be done during an appointment time. Outside of an appointment I charge my regular return visit fees for completion of paperwork. There are some situations that will require me to be in contact with you in order to complete paperwork outside of an appointment time. That is also charged at regular office rates.
